Description

Oversees and delivers patient-centered pharmaceutical care, including medication information, education, medication preparation, and distribution to ensure safe, effective, and cost-efficient medication therapy. Uses clinical judgment to provide timely input to and implementation of the patient’s plan of care based on diagnosis, while monitoring and communicating effectively with patients, families/support groups, and healthcare providers. Ensures compliance with local/state/federal regulations and privacy/safety policies; may also perform specific procedures and/or teaching duties.

Requirements

• PharmD graduate from an accredited pharmacy school

• Current registration as a pharmacist (or registered within 120 days)

• 3 years recent relevant experience

• Knowledge of medical terminology, generic/trade pharmaceutical names, pharmaceutical calculations, and laws/regulations

• Knowledge of pharmacy practice settings, including narcotic delivery and procedures

• Ability to identify and use pharmaceutical/medical terms, abbreviations, and symbols used in prescribing, dispensing, and record keeping

• Basic working knowledge of legal requirements and accreditation standards, including The Joint Commission, Title XXII, DHS, DEA, FDA, and USP

• Strong written and verbal communication skills

• General knowledge of computer applications (Microsoft Office) and pharmaceutical technology including Electronic Health Records (EHR) and EPIC
