Description

Execute leadership-level case management for complex patient care using home telehealth, health informatics, disease management, and telehealth technologies to coordinate care in the least restrictive, most cost-effective manner. Perform initial and ongoing comprehensive assessments, triage and interpret incoming patient data (vital signs, symptoms, and responses), and intervene to facilitate timely care through appropriate channels (e.g., PACT clinic, ED/urgent care, or community care). Coordinate interdisciplinary consultation and co-managed care communications with community providers and non-institutional programs, and deliver patient coaching, education, and motivational counseling to improve outcomes and reduce complications, hospitalizations, and avoidable visits.
