Description

You will provide and/or supervise direct patient care using the nursing process, with accountability for care coordination throughout the patient care continuum. The role emphasizes patient education, self-management, and customer satisfaction, while ensuring evidence-based outpatient practice. You will also support nursing education, orientation/competencies, quality improvement, and leadership in collaborative outpatient care.

Responsibilities

• Provide competent, evidence-based outpatient care to assigned patients
• Coordinate patient transitions across the continuum of care
• Deliver/enable patient and family education and support self-management after discharge
• Oversee licensed vocational nurses and nursing assistants as appropriate
• Direct nursing education, orientation, competencies, and support quality improvement/outcomes utilization
• Demonstrate leadership to improve holistic outpatient care through collaboration
• Evaluate practice continuously using best evidence
• Provide peers with informal, constructive feedback
• Foster a safe and supportive environment for professional development
• Question and improve outpatient clinical practices to ensure evidence-based care

Requirements

• U.S. citizenship (non-citizens only if VA cannot recruit qualified citizens, per VA policy)
• English language proficiency (written and spoken)
• RN education from an accredited professional nursing program (ACEN or CCNE) or meet VA-approved education/licensure pathways
• Current, full, active, unrestricted RN license
• Selective Service registration (if applicable)
• Background/security investigation
• Pass pre-employment physical evaluation
• Urinalysis testing if selected for a testing designated position
• Complete online onboarding and pre-employment requirements
• Seasonal influenza vaccination participation
• Possible probationary period
• Meet physical requirements for the role
