• Role: Program Specialist (PCMM Coordinator)

Description

Serve as the PCMM Coordinator for a VA Medical Center and its associated Community-Based Outpatient Clinics (CBOCs). You will manage PCMM program setup and operations for primary care, including provider profiles, patient assignment/recertification, electronic team and position configuration, and notifications. You will also prepare reports, train primary care leadership and mental health PCMM affiliates, and support clinic workflows such as consult routing and patient/provider change requests.

Responsibilities

• PCMM Coordination (65%)

  • Monitor and maintain provider profiles
  • Oversee and correct transmission errors
  • Review and recertify incorrect patient assignments
  • Track patients from enrollment/eligibility through NEAR reporting and assignment to a PACT
  • Create/modify PCMM team and position settings; assign provider user class based on credentials; link associated clinics
  • Set maximum panel sizes and clinical alert notification levels
  • Establish and manage preceptor to associate provider links
  • Use assignment/reassignment tools to move groups of primary care patients between teams/positions
  • Notify patients by postal mail of new team assignments
  • Coordinate clinic-wide patient/provider change requests with Nurse Managers and the Patient Advocate
  • Notify Pharmacy about relocated patients who no longer require pharmaceutical support
  • Notify other nationwide PCMM Coordinators when patients transfer outside VISN jurisdiction; ensure only qualified veterans are dual assigned across VISNs with proper CPRS documentation
  • Verify and correct national duplicate assignments monthly
  • Serve as the main point of contact for new consults/referrals and direct them to the proper provider based on panel availability
  • Monitor scheduling timeliness statistics and report problems to Primary Care leadership
  • Provide initial and reinforced training for Mental Health PCMM affiliates; ensure legitimate and maintained PCMM input
  • Participate in patient education training related to outpatient systems, clinic access, scheduling, and logistics

• Reports (25%)

  • Prepare national, VISN, and Medical Center-level reports upon request using established and ad hoc computer programming routines
  • Collect, compile, analyze, and present PCMM integrity data and develop PCMM program evaluation reports

• Training and Resource Utilization (10%)

  • Notify and train Primary Care leadership and CBOC Managers on proper FTEE formulation affecting staffing and room utilization at the VISN level
  • Create report templates and generate/update reports daily, monthly, or quarterly as requested
  • Collect, compile, analyze, and present PCMM integrity data; act as a resource for troubleshooting VistA and PCMM software applications
