Description

Provide comprehensive primary and post-acute care for geriatric patients with strong clinical autonomy. Conduct assessments, follow-ups, and chronic disease management while collaborating with interdisciplinary teams. Maintain accurate EMR documentation and develop proactive treatment plans to improve outcomes and reduce hospital readmissions.

Responsibilities

• Provide primary and post-acute care services for geriatric patients

• Conduct patient assessments and follow-up visits

• Manage chronic diseases

• Collaborate with physicians, nursing staff, and facility leadership

• Maintain accurate and timely EMR documentation

• Reduce hospital readmissions through proactive patient care

• Develop and manage preventative-focused treatment plans and quality outcomes

Requirements

• Active South Carolina Physician Assistant license (or ability to obtain)

• Board Certified (ANCC, AANP, or equivalent)

• Strong clinical judgment and communication skills

• Experience in post-acute, hospital, primary care, or outpatient settings (preferred)

• Passion for geriatric medicine and chronic disease management
