Description

You will collaborate with the healthcare team to provide medical nutrition therapy by identifying patients at high nutrition risk, assessing nutrition needs, and determining nutrient requirements based on age and disease needs. You will develop and implement nutrition plans of care, monitor outcomes, educate patients and families on appropriate food selections and behaviors, and support continuity of care across the continuum of care.

Responsibilities

• Develops and implements a nutrition plan of care for assigned patients

• Fosters continuity of care across the continuum of care

• Continuously evaluates and monitors effectiveness and outcomes of nutrition therapy

• Provides nutrition education to patients and families

• Manages delivery of nutrition care services for the assigned site of care; participates in performance improvement

• Mentors Diet Clerks and other support personnel

• Maintains professional development and competency

• Performs other duties as assigned

Requirements

• Bachelor’s or advanced degree (food and nutrition or dietetics) from an accredited institution

• Registered Dietitian credentialed by the Commission on Dietetic Registration (Academy of Nutrition and Dietetics)

• Licensed Dietitian within the State of Illinois

• Maintains dietetic registration, state-required credentials/licensure, and continuing education requirements

• Good instructional and communication skills

Preferred

• Experience in hospital clinical dietetics

• Proficiency in Nutrition Focused Physical Exams

• Prior active role with hospital or department performance improvement projects or committees
