Role Overview

The Care Transition Coordinator (CTC) connects patients, families, and healthcare partners to hospice services and serves as the primary liaison between referral sources and internal clinical teams, ensuring seamless and timely transitions of care. This role blends healthcare sales, care coordination, and partnership development to support patient outcomes and organizational growth.

Key Responsibilities

Referral Development & Sales Execution

  • Serve as the primary liaison between referral sources, patients, caregivers, and clinical teams to ensure seamless care transitions
  • Execute structured sales calls including pre-call planning, relationship building, needs discovery, and closing
  • Build and maintain strong relationships with referral partners
  • Develop and execute strategies to grow market share and drive referral volume
  • Track, manage, and document sales activity using CRM tools
  • Achieve established referral and admission goals

Relationship Building & Collaboration

  • Establish partnerships with hospitals, physicians, case managers, and community stakeholders
  • Collaborate with clinical and operational teams to support timely patient admissions
  • Represent the organization professionally across healthcare settings
  • Participate in community outreach and events

Care Coordination Support

  • Facilitate smooth transitions into hospice services
  • Communicate patient needs, risks, and updates to internal teams

Required Qualifications

  • Bachelor’s degree in Healthcare, Business, Marketing, or related field preferred (or equivalent experience)
  • Minimum of two (2) years of healthcare or related sales experience
  • Strong communication and relationship-building skills
  • Ability to manage multiple priorities in a fast-paced, field-based environment
