Job Summary

We are seeking to hire a responsible, patient, and qualified Licensed Practical Nurse to assist with reviewing patient's charts. You will review patient records to ensure that information is accurate, and that records and documents accurately reflect the conditions and treatment requirements of each patient.

Responsibilities

  • Conduct comprehensive assessments of patients' medical, social, and emotional needs
  • Provide case management services to ensure continuity of care and appropriate utilization of resources
  • Monitor and evaluate patient progress, adjusting care plans as needed
  • Collaborate with interdisciplinary teams to optimize patient outcomes
  • Oversee patient EMR to ensure that patients receive proper care
  • Complete reports to reduce patient gaps and hospitalizations

Requirements

  • LPN license required
  • Minimum of 2 years of experience in acute care or managed care setting preferred
  • Proficient in conducting patient assessments and monitoring vital signs
  • Excellent communication and interpersonal skills to effectively collaborate with healthcare professionals
  • Experience with Geriatric behavioral health preferred
