Integrated Behavioral Health Clinician (BHC) - Licensed Clinical Social Worker (LCSW)

Praxis Health is seeking a Licensed Clinical Social Worker to join integrated care teams. This role follows the Primary Care Behavioral Health (PCBH) model — consultative, collaborative, and focused on brief, functional interventions to improve overall health outcomes.

Position Summary

The Integrated Behavioral Health Consultant is a core member of the primary care team who works collaboratively with medical providers and support staff to address patients' behavioral health needs within the primary care setting. This role is embedded in the primary care clinic and provides brief, evidence-based interventions, psychoeducation, care coordination, and consultation to support patients experiencing a wide range of behavioral, emotional, and physical health concerns.

Schedule

Monday - Friday, 8am - 5pm or 4 days per week, ensuring patient contact hours are 36 per week

Minimum Qualifications

  • Master's degree in Social Work, Psychology, Counseling, or a closely related field
  • Current licensure as a Licensed Clinical Social Worker (LCSW)
  • Experience working in an integrated care setting preferred but not required
  • Strong clinical assessment and intervention skills with a focus on brief, solution-focused therapy
  • Ability to work collaboratively within a multidisciplinary team and communicate effectively with primary care providers
  • Comfortable using telehealth technology for remote patient care
  • Flexible and adaptable to changing work environments and patient populations
  • Commitment to providing culturally competent and trauma-informed care
  • Regular and punctual attendance as scheduled
