Description

Join an expanding outpatient primary care group serving Medicare patients. You will provide wellness and prevention education, perform clinical assessments, manage referrals and follow-ups, prescribe treatment, order diagnostic studies, and document care in the EMR. You will also review and sign consult notes and requests, return patient calls by end of day, support quality improvement, and oversee assigned mid-level and nursing staff.

Responsibilities

• Educate patients on wellness, prevention, and early detection; provide resources to patients and families
• Determine needed referrals and triage follow-up urgency based on exams and patient needs
• Exercise final clinical judgment; prescribe treatments and clinical drugs
• Order studies, tests, and ancillary services
• Document all services in the patient medical record; complete visit notes by end of next business day
• Review and sign consult notes and requests (lab/diagnostic/ancillary/pharmacy/health risk assessment/home health/DME) before end of next business day after placement in clinician queue
• Return patient telephone calls by end of day
• Review ongoing cases requiring attention with care management programs
• Participate in quality improvement and continuing education programs
• Consult with the Medical Director and staff as needed regarding patient care, assessment, and education
• Oversee assigned mid-level medical staff and nursing clinical staff
• Assist in resolution of patient complaints, requests, and inquiries
• Maintain confidentiality in line with federal and state regulations
• Apply appropriate diagnostic/procedural codes (CPT, HCPCS, ICD-10) and follow coding reimbursement regulations
• Initiate hospital admission/discharge arrangements and complete required paperwork
• Complete age- and disease-specific measures/testing required by NCQA/HEDIS
• Communicate effectively with patients, families, staff, and community members from diverse backgrounds
• Participate in patient retention and marketing activities as required

Requirements

• Degree in Internal Medicine/Family Practice from an accredited school
• Licensed to practice in the State of Georgia
• Board certified or eligible in primary care specialty
• 2+ years of direct work experience
• Current DEA registration and DPS certification of registration
• Outpatient practice experience with HMO/managed care (helpful)
