Description

The RCT Registered Nurse executes a streamlined process to receive, triage, and direct Veterans to timely, coordinated care options across VA internal/direct care and community care. The role applies knowledge of the referral care process, clinical review criteria, utilization management standards, documentation requirements, community care regulations, and VHA guidelines, using a national standardized triage tool.

Responsibilities

• Triage incoming referrals and confirm appropriate specialty care requests

• Discuss available care options and provide scheduling guidance

• Serve as a liaison between referring VA providers and specialty services; ensure necessary pre-work and tests are completed

• Guide Veterans through care options using the national standardized triage tool

• Comply with VA-mandated health information solutions for storing, exchanging, and tracking referral information

• Collaborate with the Office of Integrated Veteran Care to triage Requests for Services (RFS) to promote in-house access

• Use delegation of authority (if applicable) and align care delivery with key performance indicators

• Provide informal constructive feedback to peers and support professional development

• Contribute nursing perspective to interdisciplinary team discussions and support an evidence-based practice culture
