Description

Join a heart failure program focused on inpatient systems of care, outpatient population health management, and identification and follow-up of patients who need advanced procedural treatments. You will coordinate multidisciplinary care with primary care, cardiovascular specialists, and procedural teams, supported by electronic medical record population health analytics.

Responsibilities

• Create systems of care for high-quality, efficient hospital treatment of heart failure patients
• Manage outpatient heart failure population health
• Appropriately identify and follow patients who need advanced procedural treatments
• Coordinate multidisciplinary care with primary care, cardiovascular specialists, and procedural teams
• Use electronic medical record population health analytics as part of the program

Requirements

• MD or DO
• BC/BE in Cardiovascular Medicine
• Advanced subspecialty training in heart failure strongly encouraged
