Description

As an IRIS SDPC RN, you provide assessment, oversight, guidance, and education to participants enrolled in the Wisconsin IRIS Self-Directed Personal Care option. You administer the Wisconsin Personal Care Screening Tool, develop person-centered plans of care, ensure physician orders are obtained, and support care through documentation, visits/calls, and required follow-up. The role also includes submitting for prior authorization for personal care services and providing training to participants and care providers as requested.

Responsibilities

  • Administer the Wisconsin Personal Care Screening Tool and required addendums for personal care assessments and oversight
  • Document assessments and complete required oversight visits and calls per tools and Department of Health Services policies
  • Oversee groups of IRIS participants (“My Cares Groups”); develop and revise individual plans of care and obtain/confirm physician orders
  • Submit prior authorization requests for personal care services
  • Comply with Department of Health Services policies and SDPC guidelines, procedures, documentation, and program regulations
  • Provide personal care training and educational materials to participants and/or care providers as requested
  • Coordinate collateral contacts with IRIS Consultants, Long-Term Care Functional Screeners, and physicians to ensure care needs are met
  • Perform other assigned duties; overtime may be required and you may need to drive ~50% of the time for home visits

Requirements

  • At least 2 years of nursing experience; at least 1 year experience serving IRIS target groups (adults with physical/intellectual disabilities or older adults), or equivalent combination of education and experience
  • Active, unrestricted Registered Nurse (RN) license in Wisconsin
  • Associate’s degree in nursing
  • Valid, unrestricted driver’s license, reliable transportation, and adequate auto insurance for job-related travel
  • Database operation/maintenance and data entry experience
  • Teaching and mentoring skills; strong analytical and problem-solving skills
  • Strong organizational and time-management skills; ability to manage tasks independently
  • Flexibility to adapt to changing organizational needs
  • Strong written and verbal communication skills
  • Proficiency with Microsoft Office suite and/or applicable software programs

Preferred Qualifications

  • Experience providing care through the Wisconsin Medical Assistance Personal Care program (MPAC)
  • Home care/home health experience
