Description
PRN Pharmacist coverage is needed as needed during facility hours and on call after hours. The role performs clinical and operational pharmacy tasks, including accurate order interpretation, medication profile management, compounding/dispensing (including sterile, chemotherapy, and parenteral nutrition), controlled substance handling, drug therapy monitoring, and clinical consultation.
Responsibilities
Interpret medical orders (verbal and written) and transcribe them to computerized patient medication profiles accurately; maintain accurate, complete medication profiles
Compound and dispense pharmaceuticals accurately, including sterile, chemotherapy, and parenteral nutrition products
Issue controlled substances to patient care areas and maintain records as required by law
Monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose
Assist with pharmacokinetics consult service and drug dosing per organizational protocol
Review and interpret culture and susceptibility data for antibiotic appropriateness and recommend changes as needed
Detect and report suspected adverse drug reactions accurately and in a timely manner
Sustain the formulary by minimizing nonformulary procurements, using therapeutic substitution protocols, and promoting rational drug therapy selection
Provide clinical consultation and clarify orders for practitioners; suggest appropriate, cost-effective therapeutic alternatives when needed
Document all clinical activities and interventions accurately and completely
Participate in quality improvement and medication use review activities; collect data, conduct quality monitors/inspections, and maintain assigned logs and documentation
Participate in development and presentation of orientation, education, and training programs for pharmacy, medical, nursing, and other staff
Other duties as assigned
Requirements
Pharmacy degree from an accredited college of pharmacy
Nevada pharmacist license in good standing
In-patient/hospital pharmacy experience highly preferred
IV compounding experience preferred
Ability to hear, write, and speak clearly to communicate with customers and healthcare professionals
Strong customer service skills
Manual dexterity required for occasional reach, lifting, and holding of small objects
Computer savvy to learn and operate multiple pharmacy information systems
Flexibility to work various shifts
Ability to work on call or participate in an on-call rotation
May require vendor credentialing
