About the Role
Bring your nursing expertise to one of healthcare's most meaningful specialties. As a Hospice RN Case Manager, you'll provide skilled nursing care and compassionate support to patients and families during life's final chapter, helping ensure comfort, dignity, and quality of life.
This role is perfect for a registered nurse who wants to practice the full scope of nursing while building deep, meaningful relationships. You will build rapport with patients, families, physicians, facilities, and interdisciplinary team members while coordinating comprehensive care for your caseload.
Location
- Office Location: Jackson
- Main Service Area: Jackson and surrounding (Southern)
- Daily travel required within the service area to provide care in homes, assisted living facilities, and nursing homes
Hours
- Monday to Friday, 8 a.m. to 5 p.m.
- Weekend On-Call Rotation
Required Qualification
- Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
- Current State License as a Registered Nurse
- Minimum one (1) year experience as a nurse in a clinical care setting
- Reliable transportation, valid driver's license, and valid automobile insurance coverage
Preferred Qualifications
- Bachelor of Science in Nursing (BSN)
- Hospice or home health experience

- Nursing skills as defined by accepted nursing standards
- Ability to assess and respond to the needs of patients, families, and caregivers in varied settings
- Ability to function effectively in home or nursing facility environments
- Ability to manage multiple priorities, documentation requirements, and deadlines
Pay Range
$70,000—$88,000 USD
