Population Health & Value-Based Care Advance care coordination and population health initiatives. Qualifications Required MD or DO degree from an accredited medical school. Board Certification in Internal Medicine, Family Medicine, Geriatrics, Physical Medicine & Rehabilitation, or related specialty. Active unrestricted physician license (multi-state licensure preferred). Minimum 5+ years of progressive physician leadership experience. Minimum 5+ years in post-acute care, skilled nursing, long-term care, or multi-site healthcare operations. Demonstrated success improving clinical quality and operational performance.
