Overview

Optimal Care is hiring a Speech Language Pathologist (SLP) to administer speech therapy to patients on an intermittent basis in their place of residence. Services are provided in accordance with physician orders and plan of care under the direction and supervision of the Administrator/Clinical Manager. Therapy services are furnished only by or under the supervision of a qualified speech pathologist or audiologist.

Responsibilities

  • Provide a full range of Speech Language Pathology services as ordered by physician.
  • Participate with home care personnel in patient care planning.
  • Take initial history and perform initial evaluation and skilled procedures.
  • Consult with physicians regarding treatment changes and provide progress reports.
  • Complete and submit initial evaluation, treatment plan, and goals for physician approval.
  • Obtain physician recertification every 60 days if continued treatment is required.
  • Document progress notes each visit, reevaluations at expected duration, and discharge summaries.

Required Qualifications

  • Master's or Doctoral degree in Speech Language Pathology.
  • Active state licensure as a Speech Language Pathologist.
  • Knowledge of medical terminology.
  • Reliable transportation and valid automobile insurance coverage.

Desired Qualifications

  • Two (2) years' experience as a Speech Therapist preferred.
  • Home Health experience preferred.

Hours

Monday - Friday, 8:00 am - 5:00 pm

Pay Range

$81,000—$95,000 USD
