Key Responsibilities

A Speech Pathologist (SLP) administers speech therapy to patients on an intermittent basis in their place of residence in accordance with physician orders and plan of care under the direction and supervision of the Administrator/Clinical Manager. Speech therapy services are furnished only by or under the supervision of a qualified speech pathologist or audiologist.

  • Provide a full range of Speech Language Pathology services as ordered by a physician
  • Participate with home care personnel in patient care planning
  • Take initial history and perform initial evaluation and skilled procedures
  • Consult with physicians regarding changes in treatment and provide progress reports
  • Complete and submit initial evaluation, treatment plan, and goals for physician approval
  • Ensure physician recertification every 60 days if treatment continues
  • Write a progress note for each visit
  • Complete reevaluation at the expected duration of treatment
  • Prepare a summary upon patient discharge

Required Qualifications

  • Master's or Doctoral degree in Speech Language Pathology
  • Licensed as a Speech Language Pathologist in the state of practice
  • Knowledge of medical terminology
  • Reliable transportation and valid automobile insurance coverage

Desired Qualifications

  • Two (2) years' experience as a Speech Therapist preferred
  • Home Health experience preferred

8:00 am – 5:00 pm, Monday through Friday

Pay Range

$81,000—$95,000 USD
