Description
Provide personal care services to terminally ill hospice patients as needed under the direction of the RN, and assist with patient and family support through the course of hospice care and bereavement. Work as a member of the hospice team in providing hospice care according to the IDG care plan guidelines.
Responsibilities
- Provide personal care to hospice patients as needed
- Support and provide understanding to the patient’s family and during bereavement
- Perform assigned personal care functions in the patient’s residence under direction and supervision of the RN, Physical Therapist (PT), Occupational Therapist (OT), or Speech Language Pathologist (SLP)
Requirements
- High school diploma or GED
- Graduate from an approved training and competency course
- Must meet federal Home Health Aide training and competency requirements
- Minimum 2 years of experience as a nursing assistant (preferred)
- Active patient contact within past three years (preferred)
- Able to work independently and make accurate, at times quick, judgments
- Able to respond appropriately to crisis outside a hospital setting
- Acceptance of and adaptability to different social, racial, cultural, and religious modes
- Evidence of annual TB test and other state-required tests or examinations
- Physical ability for kneeling, squatting, reaching, twisting, climbing, walking, and maximal assist lifting/transferring of up to a 100-pound patient
- Sight/hearing sufficient or ability to use appropriate adaptive devices
- Tolerate exposure to odors, blood, body fluids, excrements, adverse conditions, and hazardous materials
- Current valid driver’s license, auto liability insurance, and reliable transportation
- Emotional stability and ability to cope with mental and emotional stress
