Description
Join a growing post-acute care team as an autonomous Nurse Practitioner providing comprehensive primary and chronic care services to residents in skilled nursing and long-term care facilities.
This role offers a flexible Monday–Friday schedule, dedicated care coordinator support, and the opportunity to serve as the lead clinical provider across two nearby facilities.
Providers typically round on average 35 patients per day, managing chronic conditions, transitional care, medication management, and preventive health initiatives while collaborating closely with interdisciplinary care teams. Experienced NPs are preferred, and a structured onboarding program is provided to support your success.
